How To Apply For A
Passport
It is imperative that you
read and understand all requirements for obtaining a Passport before
proceeding to complete the application or visiting the
Clerk/Auditor's Office.

IMPORTANT!! Do not sign
the application until you are instructed to do so by the issuing
agent!!
To obtain a passport
please obtain & complete the following:
1. Fees
Routine Processing
Adults (Age 16 & over)
$110.00 + $25.00 (execution fee) +$5.00 for
photos=$140.00
Children (Under age 16)
$80.00 +
$25.00 (execution fee) + $5.00 for photos=$110.00
Note:
Payment
should be made by check or
money order for the
$110.00 or $80.00
made payable to Department of State.
Payment for the execution fee and photos can be made
by cash or check payable to
Emery County.
Routine processing takes 6 weeks. Customers leaving
in less than 6 weeks need to expedite. Expedite fee is $60.00 per
application.
2. Photographs
You need
to present 2 identical passport photos size 2x2 taken within the
last 6 months.
Must be
front view with a white or off-white background. If you have the
photos taken at a passport
photo
service, please ask them to not crop the photos.
We do take the photos here in our office.
We can
also take the photos needed to obtain a visa. Photos are $5.00 for 2
per shot.
3. Evidence of
Citizenship
One of the following documents must be presented:
(Not a
photocopy, must have a seal)

Certified
copy of your birth
certificate from Vital Statistics. (This
will be returned to you)

Certificate of Naturalization or of Citizenship
-
Expired passport
-
Consular Report of
Birth.
4. Identification
One of
the following documents must be presented. To be acceptable the
document must bear your
photograph or your physical description and your signature.

Valid Driver’s License
(preferred)
-
U.S. Passport
-
Valid work or student
identification card
-
Official military
identification card
-
Certificate of
Naturalization or of Citizenship

Customers
should receive their passports within 6 weeks from
receipt of complete applications By Passport Agencies.
Important Notes
If you
are unable to present acceptable documentary "IDENTIFICATION" you
may bring a person
who has
known you for more than two years. To be acceptable, the witness
must be either an
American
citizen or permanent resident alien and must present appropriate
identification and fill out document.
To
obtain a passport for a child born in the United States to alien
parents, and to establish his/her citizenship it may be necessary to
request the parent(s) alien registration card (‘green card’) and/or
their foreign passport that was valid at the time of the child’s
birth.
Children
under age 14 require both parents’ signatures. Both parents must:
present evidence of child’s U.S. Citizenship and present evidence
that they are the parents or guardian and show valid personal
identification and sign and take oath before an authorized passport
acceptance agent. If the second parent is not available to sign, the
appearing parent must: do all of the above and present evidence that
he/she has sole custody of the child or written consent of the other
parent for the issuance of the passport or written statement
explaining why non-applying parent’s consent cannot be obtained.

A
"Notification of Birth" is a notice of the birth NOT an official
document and can not be accepted as evidence of citizenship. The
"Certificate of Birth" given by the hospital and the "Baptism
Certificate" are not acceptable evidence of U.S. Citizenship.
A
passport that has been altered or mutilated CAN NOT be accepted as
identification.
